Ask for the whole bill.
A medicine price can leave out much of the plan. Get the first bill and the ongoing bill in writing.

The medical visit
Is the visit charged once, charged each time, or included in a membership?
The medicine
Is the medicine included in the price? What is the exact medicine covered by that price, and what form does it take: pill, shot or something else?
Tests and follow-up
What is included, what is extra, and who pays for a local test?
The continuing charge
What happens after any introductory offer? What is charged if you cancel or the plan changes?
A dated cash-price example
Self-pay means paying cash outside insurance.
This example is for semaglutide (Wegovy) injection pens: devices supplied with medicine inside for giving a shot.
As of September 2026, new patients paying cash were offered their first two monthly boxes at $199 per box, with four pens in each.
The offer covered only the 0.25 or 0.5 milligram (mg) pens; mg tells you the amount of medicine in each shot.
After those first two boxes, a monthly box cost $349 for pens containing 0.25 to 2.4 mg, or $399 for the Wegovy HD pens containing 7.2 mg.
People with Medicare could pay the manufacturer’s cash price, but could not use its copay card, as of September 2026.
A copay card helps pay your share of a medicine cost billed through insurance.
Ask the pharmacy whether your prescription qualifies and what your next box will cost.
These amounts describe prices, not advice about how much medicine to take.
The introductory terms ran through December 31, 2026.
These are medicine-only prices under advertised terms, not a quote for a complete care plan.
Cash payments under these terms do not count toward an insurance deductible.
Your deductible is what the plan requires you to spend before it begins paying covered costs.

Check coverage for the actual treatment
The medicines covered by your plan appear on a list called its formulary.
Prior authorization means getting the plan’s approval before it pays.
Name the product and the reason for it
For Medicare, coverage can depend on why a medicine is prescribed, the plan’s drug list and its approval rules, as of September 2026.
Ask for your own amount
Ask what you would owe at the pharmacy and whether the visit and any tests are covered.
Check the kind of savings offer
Your portion of a cost covered by insurance is called a copay. A copay card offers help with that share.
Patients covered by Medicare, Medicaid or the military cannot use copay cards for semaglutide brands Ozempic/Wegovy or tirzepatide brands Mounjaro/Zepbound, as of September 2026.
A copay card and a self-pay offer have different terms; the card exclusion does not mean people with government coverage cannot use a cash price.
Medicare has a temporary program for some people
Part D is Medicare’s prescription drug coverage.
Some weight-loss medicines are covered by a temporary Medicare program.
As of September 2026, the program included semaglutide (Wegovy) shots and tablets, and tirzepatide (Zepbound) only in its KwikPen injection pen.
Other Zepbound pens and vials were not included.
As of September 2026, eligible patients paid a $50 monthly copay through the program, which ran from July 2026 through December 2027.
Whether you qualify depends on age, weight, other health conditions, your Medicare plan and its past payments for these medicines.
For example, people with type 2 diabetes must seek payment through their regular Part D plan instead, as of September 2026.
That program’s $50 copay is excluded from the usual Part D yearly spending cap, and low income does not lower it.
Ask the person treating you to check the official eligibility page with you and help request the program’s approval.
Having Medicare alone does not mean you qualify.
Ohio also limits certain online-visit fees
A facility fee is a charge separate from the medical visit fee.
As of September 2026, Ohio bans a separate facility fee for an online visit and charges for the treating professional’s equipment.
This does not cap the fee for the medical visit or the medicine price.
